We are in-network for Cigna, United Healthcare, and certain Blueshield PPO plans (Note: not the Blueshield plans that are contracted through American Specialty Health (ASH).
For out-of-network plans, we can provide a superbill (itemized receipt) that you can submit to your insurance that will go towards your deductible and possibly reimbursement.
We accept HSA and FSA payments.
*Please submit your insurance information 72 hours before your first appointment. All information is kept strictly confidential and will only be used for verification purposes with your insurance company.
Please Note Late Cancellation Policy - Any cancellations without 24 hour notice are subject to the full cash fee of $230 or $145 regardless of insurance or not as it takes away a slot for someone else who can really benefit from the treatment.
