If you would like my team to verify your insurance benefits, please fill out the insurance verification form at least 2 business days prior to your appointment.
*This is not a guarantee of benefits or reimbursement. We highly recommend the patient calling or checking their insurance portal to confirm their eligibility and benefits for your services as well*
For out-of-network plans, we can provide a superbill (itemized receipt) that you can submit to your insurance that will go towards your deductible and possibly reimbursement.

Please Note Late Cancellation Policy - Any cancellations without 24 hour notice are subject to the full cash fee of $240 or $150.
